Motion IQ Physical Therapy accepts Medicare and is an out-of-network provider with PPO health insurance plans. Many insurance plans provide benefits for physical therapy services provided by out-of-network providers. Health insurance plans, policies, and terms vary greatly, and each person’s health care needs and expenditure is unique. Motion IQ Physical Therapy does not accept HMO or Medi-cal.

Filing Insurance Claims

Motion IQ Physical Therapy files our patients’ primary insurance claims as a courtesy. Please note that while submits claims on your behalf, you are still responsible for ensuring that the insurance provider properly processes your claims.

Payment

We require full payment when services are rendered. We all major credit cards including MasterCard, Visa, Discover and American Express. We also accept payments from your HSA/FSA account.
